If you are a new client and have scheduled your first session, CONGRATULATIONS on taking the first step towards achieving your goals!
It is important for you to be fully informed about the therapy process and for your therapist to be fully informed about your health history, individual needs, and personal goals. Please complete ALL steps below to prepare for your first session.
STEP 1: Complete the Intake Questionnaire. This is a web-based questionnaire that will submit automatically to Jessica for her review and she will utilize this information to prepare for your first session.
**This must be completed at least 48 hours before your first session to maintain the appointment as scheduled.**
STEP 2: Check your email for a message from TherapyPortal.com. You will find a link to connect to the PHS Client Portal and once connected you will find important documents for you to review and electronically sign. This information is very important to ensuring that you are informed about the process of therapy, your rights as a client, confidentiality (including limitations), and the business and billing practices of PHS. For minors, this MUST be signed by the legal guardian AND the minor.
Please read and sign this packet prior to your session. You may ask questions at the session if needed.
STEP 3: If you plan to use insurance for services, you must complete the Insurance Information packet. As mentioned in the Intake Packet, you are strongly encouraged to contact your insurance company to most accurately determine what is covered by your policy.
This information needs to be provided 48 hours prior to the session if you want insurance benefits to be verified prior to beginning services.
STEP 4: It is highly encouraged that you permit your therapist to collaborate with your other providers (i.e. your primary care doctor or Psychiatrist). Please complete the Authorization for Release of Information form for your Primary Care Doctor and/or Psychiatrist. You may also complete the form if you would like your therapist to speak with any other agency or person about your care, on your behalf.
