If you are a new client and have scheduled your first session, CONGRATULATIONS on taking the first step towards achieving your goals!
It is important for you to be fully informed about the therapy process and for your therapist to be fully informed about your health history, individual needs, and personal goals. Please complete ALL steps below to prepare for your first session. Please note that in order to maintain your appointment as scheduled ALL intake paperwork must be completed at least 48 hours prior to your appointment.
To complete the intake paperwork:
Check your email for a message from TherapyPortal.com. In this email you will find a link to connect to the PHS Client Portal.
Once connected you will find important documents for you to review and electronically fill and/or sign. This information is very important to ensuring that you are informed about the process of therapy, your rights as a client, confidentiality (including limitations), and the business and billing practices of PHS. The health history information allows your therapist to best prepare your appointment to meet your needs. Please ensure that you complete all forms provided to you in the Client Portal.
**If you are completing the forms on a tablet or mobile device you may need to download the free app Adobe Reader from your app store to allow you to save filled documents to your device and then upload them to the Client Portal.
***It is best to complete the paperwork approximately 1 week prior to your appointment so that if you have any difficulties you can contact the practice for assistance.
